Torfaen has two Neighbourhood Care Networks (NCNs). The Torfaen South Cluster is made up of six GP practices working together to improve care for local patients across South Torfaen.

NCN visit from Judith Paget 🗣️
Judith Paget, Chief Executive of Aneurin Bevan University Health Board, attended a Torfaen South NCN meeting.
Following the meeting, she contacted Dr Amanda Head to say she was:
“Truly impressed with the number of colleagues at the meeting, the range of partners present and the discussions. You are clearly building excellent relationships and a shared sense of purpose focused on the needs of South Torfaen residents. Thank you – I left feeling positive and motivated about the work you are leading.”
Judith also expressed interest in attending further NCN meetings across Gwent.
Awards and recognition 🏆
NCN plans highlighted the need to improve primary care for patients living in care homes.
It is excellent news that work led by a Lead Care Home Nurse, supported by NCNs and nursing homes, was shortlisted for the 2016 NHS Wales Awards under the category: Improving Quality using Improving Quality Together (IQT) methodology.
Using IQT Silver methodology, nurses worked with care homes to improve:
- Early identification of deteriorating conditions
- NEWS scoring and clinical observations
- DNACPR decision-making
- Advance Care Planning
IQT Silver Award winners included:

- Millheath Nursing Home – Newport North NCN
- Crick Care Home – Monmouthshire South NCN
