Antenatal Clinic

Friday mornings with a midwife.

We provide full maternity care, both antenatal and postnatal, but we do not undertake home deliveries.

Phlebotomy (Blood Taking) Clinic

Fasting Bloods - Monday to Friday 09:00 – 09:25 (by appointment).

Non-Fasting Bloods - Monday to Friday 09:30 – 11:30 (by appointment).

Cervical Smears

The Health Authority policy is that a smear is recommended every three years between the ages of 25 and 49 and every five years between the ages of 50 and 64. Twice weekly Nurse led clinics are held.

Child Health Clinic

This clinic is organised by the health visitor and is held in clinic at Tudor Road.

6 weeks baby examinations are held in Clark Avenue surgery every Tuesday between 14:30pm and 15:30pm. This clinic is for well babies. Any child with an illness will be seen during normal surgeries.

Contraception

We provide a range of contraceptive services including IUDs (coils), implants and contraceptive injections, which are described in NICE (National Institute for Health and Clinical Excellence) guidance on Long Acting Reversible Contraception.

COPD (Chronic Obstructive Pulmonary Disease) Clinic

My Health Online Now Available for Use at Clark Avenue

You can use the Online service to book & cancel appointments, order repeat prescriptions and send forms to the practice.

How does My Health Online work?

My Health Online connects to your practice’s clinical system. Think of it as a window into your practice’s services. For My Health Online to work, your practice must set it up to allow patients to sign in.

Is My Health Online a free service?

Yes, My Health Online is free to practices and patients.

Is my information secure?

Yes, all information sent to or from My Health Online is secure. All information is encrypted and protected using the highest internet security standards.

When is the service available?

The service is typically available at all times. Your practice might stop the service for short periods during the night so that they can do essential maintenance.

Registering for the service

Registration using a PIN and Access ID

To get access to all the My Health Online features, you need a PIN and Access ID. To get these, visit your practice with photographic identification (such as a driver license or passport) and ask to be registered for My Health Online. The practice will give you your PIN and Access ID. Go to the My Health Online website. On the login page, click Create an account (this will be renamed to Registration in the future) and enter these details when asked. The system will guide you through the process of setting up a My Health Online account.

Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to make this initial appointment as early as possible - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Non-NHS Services

Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

• Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
• Insurance claim forms
• Passport signing
• Prescriptions for taking medication abroad
• Private sick notes
• Vaccination certificates

The fees are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

Hospital Transport Services are provided to help patients get to their hospital or clinic appointment. Patients who need ambulance transport will now book their transport to hospital directly through a Transport Booking Centre.

This is a separate service to the emergency ambulance service - if you need an ambulance for a medical emergency you should always phone 999.

When you receive your appointment letter from the hospital you should contact the Transport Booking Centre on the freephone number - 0800 3282332.

The booking Centre is open from 08:30 to 18:00 Monday to Friday, but not on public holidays.

Remember - If your appointment is changed for any reason, please tell the Transport Booking Centre.

You will need to telephone the Transport Booking Centre at least one week before you are due to attend. The Booking Centre will not take bookings more than 3 months in advance.

If you are eligible for transport, it will be booked for you and you will be given a booking number. Please make a note of this number as you will need it if you have any queries.

Receive Free Appointment Reminders - You can now received appointment reminders for certain clinics and health information via text.  Please hand in your mobile number to the surgery reception desk.  There is no charge to you the patient for text messages sent to your mobile from the surgery.  If you wish to stop receiving text messages from the surgery at any point please contact the surgery to opt-out.
