Welsh GP Record
Your Welsh GP Record is a summary of important information taken from your full GP medical record. It can be accessed by selected health professionals in Out of Hours services and some hospital departments.
The record may include:
- Your name, address and contact details
- Current medication and medication prescribed within the last two years
- Allergies or adverse reactions
- Current problems or diagnoses
- Results of recent tests from the previous year, such as blood tests or X-rays
It does not include any private discussions you may have had with your GP.
Management of Records
Records and Information Management are key parts of Information Governance. They guide how information is stored, protected and used safely within the practice.
Good records management helps ensure we have the right information available at the right time to make safe and effective decisions about patient care.
Organisations must create and manage records appropriately in line with legislation including:
- UK General Data Protection Regulation (UK GDPR)
- Data Protection Act 2018
- Freedom of Information Act 2000
Records within an organisation may include administrative records, paper and electronic documents, emails, audio and video recordings, X-rays and CCTV footage.
